Bridge Community Based Solutions will provide Individual Residential Services and Supports (IRSS) individuals with intellectual and developmental disabilities (IDD) 24/7 daily in a variety of residential settings. The goal is to ensure each individual receives personalized care that promotes independence, safety, community inclusion, and a high quality of life. We uses a variety of living arrangements individually designed to meet each person’s needs.The individual may live in a home owned or leased by the agency, their own home/apartment, or a Host Home.
Services Provided Include:
- Assistance with Daily Living Skills: Support with hygiene, grooming, bathing, dressing, and meal preparation.
- Health and Safety Monitoring: 24/7 supervision tailored to individual needs, medication administration, and health care coordination.
- Community Integration: Support in accessing community activities, recreational opportunities, and building social relationships.
- Skill Building: Assistance with developing functional life skills, such as budgeting, communication, and transportation.
- Behavioral Support: Implementation of individualized behavior plans in collaboration with a behavior specialist when needed.
- Advocacy and Person-Centered Planning: Ensuring each individual’s voice is heard in decisions that affect their lives through person-centered plans and goals.
Residential Settings Include:
- Host Homes – Individuals will live in the private home of a qualified and trained Host
- Home Provider who offers 24/7 care, supervision, and support in a family-like
- environment.
- Personal Care Alternatives (PCA) – small group homes or individual living situations with support.
- Family Caregiver Model – support provided by a trained family member.
- Independent Living with Support – for individuals with higher levels of independence but still needing some assistance
