At Bridge Community Based Solutions, we believe that every person regardless of ability deserves the opportunity to live a meaningful, independent life with dignity and connection to their community. That’s why we are proud to offer Individual Residential Services and Supports (IRSS) and Supported Community Connections (SCC) as part of our approved services under Colorado’s Home and Community-Based Services (HCBS) waivers.
What Are IRSS Services?
IRSS (Individual Residential Services and Supports) help individuals with intellectual and developmental disabilities live as independently as possible in a home setting. Through IRSS, we provide:
- Personalized support based on each person’s goals and needs
- Assistance with daily living activities (like cooking, bathing, and medication management)
- Safe and comfortable living environments through host homes, family caregivers, or personal care alternatives
- Life skills training to promote independence and confidence
- 24/7 support or as-needed care, depending on the person’s needs
We work closely with individuals, families, and case managers to create a supportive, person-centered plan that encourages choice, independence, and stability.
What Is SCC?
Supported Community Connections (SCC) are services that help individuals with disabilities participate more fully in their local communities. SCC helps reduce isolation, increase confidence, and promote inclusion by providing support in accessing:
- Local parks, recreation centers, and libraries
- Volunteer opportunities and community events
- Group or one-on-one activities tailored to interests and goals
- Cultural and social integration experiences
Our trained staff accompany and support individuals as they explore their communities, make new connections, and develop skills that foster long-term engagement.
